Chronic Disease Integrated Care Program

The Chronic Diseases Integrated Care Program aims to address the prevalence of chronic diseases such as Diabetes, Coronary Heart Disease (CHD) and Chronic Obstructive Pulmonary Disease (COPD) through General Practice.

The program involves maintaining a database of chronic disease patients who are recalled to their GP at six and twelve month intervals. The patients are also provided with a resource folder to help them “take control” of their disease management.

The program aims to:

  1. to promote better clinical care through patient recall, monitoring of clinical practice and monitoring of local health outcomes
  2. To increase consumer satisfaction with their illness management and service provision.
  3. To assist GPs to provide a more comprehensive service to people with Chronic Disease (including those who are geographically isolated) by developing professional networks and establishing a formalised shared care arrangement with organisations who currently provide local Chronic Disease services.
  4. To assist GPs to play a key role in, and to enhance their management of, patients with Chronic Disease

Good for Kids, Good For Life Program

Good for Kids, Good for Life is a childhood obesity prevention program that targets children 2-15 to decrease obesity in this population, which is currently at an alarming 25%. The broader program started in 2008, and is being run by Hunter New England Area Health Service. The Barwon Division of General Practice is responsible for promoting this program to GPs.

The main messages of the program are:

  1. Get Active, Get out and Play – 30 mins of physical exercise a day.
  2. Drink H20 – Think water first
  3. Eat more fruit and veggies – go for 2 fruit and 5 veggies.

The aim of this program for GPs is to promote opportunistic assessment of Body Mass Index to identify and track children at risk of childhood obesity. This can be done in conjunction with the 4 year old Healthy Kids Checks, but also as part of regular consultations. GPs can also be involved in providing education to parents on healthy lifestyles for their children, and referring on to appropriate professionals such as dietitians or exercise professionals.
